DBS Application Form Guide
The DBS application form is a relatively straightforward and simple form which should only take you 5 -10 minutes to complete. However there are a couple of very important points to remember which will ensure the process runs smoothly and that you do not waste your time by bringing in IDs which cannot be accepted. This guide covers the two areas which cause the most confusion:-
- Verification Documents
- Current and Past Addresses
A lot of students come to have their ID verified and find they either do not have valid forms of ID or have ID which does not match their ‘current’ address so please do read this guide carefully
You should have already received an activation email from First Advantage Online Disclosures so that you can start completing the online application form. The organisation pin that is given in the email and that you will you need again when you come to pay for your application is 139308. Please follow the link in this email to start your application.

Valid ID documents
You will need to have 3 valid forms of ID in order to submit your application. All ID documents must be shown at the Verification session and must be originals (photocopies or a print from an online document are not accepted) The UK government has a list of documents that can be accepted as a valid form of ID.
One or two of these must come from the list below (Group 1 docs) :-
Your remaining one or two IDs must come from the Group 2 list below. One of these must be dated within the last 3 or 12 months (as in the lists below)
Current Address and History
A five year address history is required which includes your current and previous addresses. Your current address is the one that your DBS certificate will be sent to and which must appear on one of your ID documents. For the majority of students this will be the address on their bank statement or driving licence.
For UK students this may mean you will use your ‘home’ address rather than your university address as your ‘current’ address. If this is the case you should enter your university address as a previous address under your Address History. This will then expect you to enter a leave date for this address so enter the current date even though you still live there.
When you have Completed your Application Form
Once you have completed and submitted your application and you have all 3 of your original ID documents with you in Reading you need to book to come to one of our drop-in DBS Verification sessions so that we can check your ID documents.
These drop-in sessions should only take about 10 – 15 minutes of your time and you can book a place when you log in the Student Portal on the Students in School website:-

Once your ID documents have been verified and your application has been submitted for processing you will need to login to the FADV website again to pay the £7.20 administration fee
Your certificate will be posted to you ‘Current Address’ and will usually arrive within 7 – 10 days.
Keep your certificate in a safe place as you will not be able to get a copy if you lose it.
